A sample has been taken from fluid around your testicle with a needle.
The testicles are located outside the body, and are covered by skin. That is the scrotum. Sperm are produced in the testicles and then transported into the epididymis. Fluid had accumulated around your testicles. The fluid taken from it can then be examined in the laboratory.
