You have mental problems. Your movements have changed as a result of mental processes. There is a physical reason for this.

You have mental problems. Your thinking, perception and feelings have changed as a result. There are physical reasons for your condition.

You have a particular type of symptom. With this, it may be that one cannot move although fully conscious. One may also adopt unusual attitudes and maintain them. However the opposite, too, may occur. One may move about a great deal and make pointless movements.
