Your thoughts, perception and mood have changed.

You have a mental disorder. Your thoughts or perception have also changed. You may then also misjudge reality. For example, you may think that you are being persecuted. You may also have sensory illusions. For example, you then hear or see things that are not really there. Your own thoughts might keep changing or be disconnected. The disorder is also accompanied by a change in mood. You may also have other symptoms.
